Minor Participant Policy
Parent/Minor Waiver
packets must be filed before minors will be allowed to
participate.
While we will do our best to
work with you, we ask that you complete these things during the week or
before your event day to avoid any problems.
Minor will
not be allowed to participate without proper waiver and ID on file. No
Exceptions!
Single Event
Waiver
Parents will be required to accompany a
minor to every event.
Parent and Minor must provide
identification and sign a waiver at every event.
Parent and minor’s last name must match
or proof must accompany forms.

All-Season Waiver
NEW Participants
New Participants need to
have their Yearly Waiver signed only once at the beginning of the
season. This can be done in one of three ways.
1. Have both birth parents
sign
2. Have one birth parent and
one Legal Guardian sign
3. Have Sole Custodial
parent sign.
You must also provide
the minor’s Birth Certificate and parent’s identification.
If you have legal
guardianship you must provide legal documents proving that.
RENEWING Participants
ONE birth parent and minor sign the waiver at the beginning of the
year.
In order to be a
Renewing participant, your previous year's file must be complete.
***All
waivers, forms, and IDs must be originals. No copies will be allowed.***
***All
Waivers and IDs must be signed and copied in front of RMR Management or
in front of a notary public.***
After
all the requirements are met, a photo will be taken of the Minor and a
“Minor Card” will be issued. This “Minor Card” must be present every
time the Minor participates in an event. Put it in a safe place. If the
card is lost, contact the office to have another card issued as soon as
possible.
