Single Event
|
| a. One parent or Legal Guardian must be
present to sign the waiver |
| b. A government issued form of ID |

• This needs to prove the
identity and Date of Birth of the Minor. |
| c. All forms must be originals. (No Copies) |
|
• Forms include: waiver and ID |
| d. Minor Drivers: Must also have a Statement of Health waiver filled out and their Social Security Card to get paid. |

All Event
|
| a. Both Birth Parents or Legal Guardian must
be present to sign the waiver. |
|
• If both parents are not
available to come to the track, you may pick the waiver up
and have it notarized. |
| b. A government issued form of ID |
|
• This needs to prove the
identity and Date of Birth of the Minor. |
| c. All forms must be originals. (No Copies) |

• Forms include: Yearly waiver
and ID |
| d. Minor Drivers: Must also have a Statement of Health waiver filled out and their Social Security Card to get paid. |
| e. All waivers must be completed every year.
Renewing minors are only required to have one parent present
with id to renew their all event packet. |
After all the requirements are met, a photo will be taken of
the Minor and a “Minor Card” will be issued by following week.
This “Minor Card” must be present every time the Minor attends
a race in order to enter the restricted areas. Please put it in a
safe place. If the card is lost please contact the office to have
another card issued as soon as possible.
While we will work with you, we ask that you try to complete these
things during the week or before your event day to avoid any
problems.
